Payment in full is required at the time of service.

As a courtesy, we can provide a superbill upon request, which you may submit directly to your insurance company for possible out-of-network reimbursement.

• Payment to Serenity Peak Health is required regardless of whether your insurance provides reimbursement.
• Reimbursement is determined solely by your insurance plan and is not guaranteed.
• Patients are responsible for submitting superbills, communicating with their insurance company, and following up on the status of any reimbursement.
• Serenity Peak Health does not submit insurance claims, verify out-of-network benefits, appeal claim denials, or communicate with insurance companies regarding reimbursement.
